Modivcare is the MATP provider for Philadelphia County. Some families may remember the company by its former name, LogistiCare.
In many Pennsylvania counties, MATP requires a standard application. According to Community Behavioral Health’s Philadelphia MATP information, Philadelphia residents do not need to complete an application for MATP services.
Modivcare must still confirm Medical Assistance eligibility and confirm that the trip is for a covered service.
This is shared, non-emergency medical transportation. It does not replace 911, an ambulance, or stretcher transport. Riders may travel with other passengers. Transportation is usually arranged in the least costly and most appropriate way based on need.

For Philadelphia, Community Behavioral Health notes that MATP can be used for Medical Assistance-covered services, including behavioral health care, mental health treatment, drug and alcohol treatment, physical therapy, occupational therapy, and pharmacy trips.

The rider must receive Medical Assistance and have an unmet transportation need. MATP transportation must be for Medicaid-covered healthcare.
In Philadelphia, contact Modivcare to register or schedule. Have the rider’s ACCESS card number, date of birth, phone number, pickup address, appointment address, appointment time, provider name, provider phone number, mobility needs, and escort needs ready.
MATP generally picks riders up at the curb. If the rider has a disability or limitation that keeps them from getting to the curb, MATP must provide door-to-door service when the need is medically verified.
